Dental Care Advantage

Member Discounts on Dental, Vision, Prescription, Hearing and Chiropractic Care

Farm Bureau members can now participate in the Dental Care Advantage (DCA)/ DenteMax program for only $40.00 per year per household. This is a significant savings from the usual rates of up to $124.95 per year, per family.

The DCA program provides savings of 20% to 55% on dental services when seen by a participating dentist or dental group.

The dental network provider is Dentemax, who has 1,508 dentists in Arizona.

The new dental plan features:

  1. No deductibles;
  2. No waiting periods;
  3. No annual lifetime benefit maximums;
  4. No claim forms; and
  5. No prior authorization required.

Also included at no extra charge to Farm Bureau members are discounts on:

  • Vision care discount program through EyeBenefits
  • Hearing care discount program through Epic Hearing healthcare
  • Prescription drugs discount program through Caremark
  • Chiropractic discount program through Comprehensive Health Group
