Member Discounts on Dental, Vision, Prescription, Chiropractic Care

Farm Bureau members can now participate in the Dental Care Advantage (DCA) program for only $36.00 per year per household. The DCA program provides savings of 20% to 55% on dental services when seen by a participating dentist or dental group.

The new dental plan features:

  1. No deductibles;
  2. No waiting periods;
  3. No annual lifetime benefit maximums;
  4. No claim forms; and
  5. No prior authorization required.

The DCA membership also offers Farm Bureau member discounts on vision care, prescription drugs and chiropractic services.

The Card ID is a 3 or 4 digit security code that is printed on the back of your Visa, MasterCard, Discover or American Express Card. The number appears on the authorized signature after the last four digits of your account number.

If you are using an American Express Card, then the number is 4-digits and is located on the FRONT of the card slightly above and to the right of the end of your card number.

The Authorized Signature may contain your entire account number or just the last four digits of your account number. Either way, the Card ID will appear after the last four digits of your account number. The Card ID is used to verify that you have possession of the credit card you are attempting to use.
